The iliotibial band (IT band) syndrome is the most common cause of knee and hip pain in cyclists. This
band,
which
runs on the outside of the leg from the hip to the knee, is a thick fibrous band
of tissue. The band can become tight and pain is caused when the band rubs over
the bones of the hip (greater trochanter) and/or the knee

and/or jumping and could cause a very painful knee condition called patellar tendinitis,
Iliotibial Band Syndrome or Patellofemorial known as "cycling's knee". The

3. X-rays are rarely abnormal
but can offer evidence that other knee problems are not causing the pain. X-rays of the
patella may show that it is deviated to the outside.

(IT) band (a ligament that runs along the outside of the thigh) is rubbing against the

A proposed hypothesis: there is no such thing as
Tendinitis or Iliotibial Band Syndrome. Or,

Tendonitis such as that of the Achilles, Iliotibial Band Syndrome. lateral elbow, and rotator cuff tendons is a common presentation to family practitioners and various medical specialists.
Most currently practicing general practitioners were taught, and many still believe, that patients who present with overuse tendonitis have a largely inflammatory condition and will benefit from anti-inflammatory medication.
Unfortunately this dogma is deeply entrenched.
Ten of 11 readily available sports medicine texts specifically recommend non-steroidal anti-inflammatory drugs for treating painful conditions like Achilles, Iliotibial Band Syndrome. and patellar tendonitis despite the lack of a biological rationale or clinical evidence for this approach.
Instead of adhering to the myths above, physicians should acknowledge that painful overuse tendon conditions have a non-inflammatory cause.
A critical review of the role of various anti-inflammatory medications in soft tissue conditions found limited evidence of short term pain relief and no evidence of their effectiveness in providing even medium term clinical resolution of clearly diagnosed tendon disorders.
